If you think your statement is wrong, or if you need more information about a transaction on your statement, you must notify us in writing at 645 E. Missouri Ave., Ste. 108, Phoenix, AZ 85012 as soon as possible, but no later than 60 days after the error appeared on your statement. You may call us, but if you do we are not required to investigate any potential errors and you may have to pay the amount in question.
In your letter, please provide the following information: Your name, account number, date and dollar amount of the suspected error, description of the error, and explanation of why you believe there is an error.

Children under 13 should not send any information about themselves to the Bank, including information like your name, address, or email address. The Bank does not knowingly collect personally identifying information from children under the age of 13. In the event that the Bank learns that such information has been received from a child under 13 without parental consent, the information will be deleted from the Bank’s database as soon as reasonably possible.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.
